NJ FamilyCare Application Assistance

NJ Family CareThe Family Center staff is trained to help complete the NJ FamilyCare applications.

To qualify for NJ FamilyCare, applicants must meet the following criteria:

  • New Jersey resident
  • U.S. citizen or qualified immigrant whose documents allow them to live here permanently
  • 18 years of age or younger, or a parent/guardian of a child 18 years of age or younger
  • Uninsured for at least three months prior to application
  • Within 2009 income eligibility guidelines (at or below 200% of federal poverty level). Click here for a chart detailing the Gross Income Guidelines.

Residents may obtain assistance at the Family Center nearest to their home and are encouraged to call for an appointment. Family Centers are located in Egg Harbor Township, Hammonton, Pleasantville, and two in Atlantic City at 1700 Marmora Avenue and 411 N. New York Avenue.

NJ FamilyCare applicants must provide proof of income (2 consecutive pay stubs, employer offer letter, etc.), proof of citizenship or qualified immigrant status (official passports, Visa, permanent resident card, etc.), proof of other insurance, if applicable.
